To the Honorable the Legislature of West Virginia
The undersigned your petitioner would most respectfully represent to your Hon body that in the year 1861 he was a member of Captain Cumberland Adkinsons[sic] Company of Peytona Home Guards of Boone County. That in the year 1866 or about that time said company was paid by the State for their services.
Your petitioner would further represent that at the time said Company was paid off he was absent from this State and that some person representing himself to be your petitioner , the undersigned (George P. Cook) was paid the money due your petitioner amounting to $183.33.
In view of the forgoing facts your petitioner would much respectfully ask your Hon body to pass a bill [illegible] making an appropriation for his relief and as in duty bond he will later pay to

Personally appeared before the undersigned a notary public in and for the county state aforesaid. George P Cook whose name is signed in the forgone petition and made oath that the statements and allegations outlined in said petition are correct and true to the best of his knowledge and belief and as he truly believes.
