Captain (Alfred F.) Cumpston's commission is filled out for the Fifth Regiment of Mounted Volunteers, when it should have been for the 5th Regiment Infantry.
It also should date from the 10th day of October, as that was the day he was mustered into service with his company.
Adjutant (Thomas N.) Davey in writing for his commission omitted to mention the date. The Paymaster would not pay him, only from the date of the commission.
