I have the honor to forward to you the acceptance and oath of 1st Leiut Bamabas S. Cook 126 Reg. West Virginia Militia------

He was wounded some two months ago and since that time has been in this hospital with a fractured thigh-------

and is recovering------

He received no orders with his commission and mail orders from your office-----
